Forms
We collect information about your camper so we can ensure
our staff team is adequately prepared to attend to the
campers needs. Please review the
directions below and submit
forms to us at your earliest convenience.
Please submit all forms to us as early as possible. Our cut
off date is June 1, 2010. After
June 1, we cannot guarantee that your camper’s forms
will make it to our facility in time. We may ask you to
complete a second copy as a result.
Please follow these steps to retrieve
the appropriate forms:
1. Find the camp you’ve
registered for.
2. Under your camp, click on the name of the program you’ve
registered for. You can find the program name on your financial
statement/receipt under Camp Information and Program. Note:
If you’ve registered for a bus or
an in between weekend, you’ll see
it listed under Program as well. You do not need
any special forms for these programs.
3. Click the links to each of the forms listed
under your program.
4. Complete the forms and return them to us by
e-mail (preferred) at summercamps.registrar@guidesontario.org.
You may also mail them at Summer Camps Registrar, 14 Birch
Ave., Toronto, ON, M4V 1C8 or fax to 416-920-1440.
