SAFE GUIDE FORMS IN ONTARIO
Going Camping? Be sure to complete all Level 3 & 4 Safe Guide forms at least 3 weeks prior to your activity date to safeguide@guidesontario.org to allow for volunteer Assessors to review and approve your forms. Please be certain to use the most current version of the Safe Guide forms available at http://forms.girlguides.ca/default.aspx.

If the camp or facility is handling its own bookings directly:
Your inquiry will be forwarded to the Booking Contact at that location for
follow-up and processing. Please feel free to contact that person directly.
Their contact information is shown on the Camp Profile.

If the Provincial Office is handling your booking:
Upon receipt of your fully completed Camp and Facility Booking Request
you will be sent a Rental Agreement for signature.
